Animal studies have raised concerns about lanthanum deposition in a number of tissues including liver, lung, and kidney. There has been no evidence of similar tissue deposition in humans to date. Sevelamer has been associated with the development of a metabolic acidosis and, in general, should be used sparingly and cautiously in the CKD population. Active vitamin D therapy is used to further suppress PTH into the target range. Calcitriol, the naturally occurring active form of vitamin D, suppresses PTH and reverses renal osteodystrophy. While both oral and intravenous forms have been shown to be effective, calcitriol is limited to a narrow therapeutic index by side effects.Vitamin D receptors are present on a variety of tissues besides the parathyroid gland, including the intestines, kidneys, bone, immune cells, skin, heart, and brain. As a result, calcitriol stimulates intestinal reabsorption of calcium and phosphorus and may cause hypercalciuria, hyperphosphatemia, and an elevated calcium-phosphorus product. Vitamin D analogs such as paricalcitol 19-nor1, 25 OH ; 2D2 ; and doxercalciferol 1 OH ; D2 ; have. INTRODUCTION Osteoporosis is characterised by low bone mass and micro-architectural deterioration of bone tissue, leading to enhanced fragility and risk of fracture. Primary osteoporosis is an age-related process, which can affect both sexes, but women are at greater risk as they achieve a lower peak bone mass than men and bone loss is accelerated following the menopause. Common fracture sites include the long bones distal forearm and the neck of the femur ; and vertebrae. Such fractures can cause pain, deformity e.g. kyphosis and height loss ; and disability, although most vertebral fractures are asymptomatic. The definition of osteoporosis is based on having a low bone mass, measured as the bone mineral density BMD ; . Osteoporosis is defined as a BMD 2.5 or more standard deviations SD ; below the young adult female mean T score less than 2.5 ; . Patients are considered to have osteopenia if the T score is between 1 and 2.5. However, a reduced BMD is only one of many risk factors for fracture, and BMD alone is a poor predictor of fracture risk. Others risk factors include previous fragility low-impact fractures, certain medical conditions and a family history of osteoporosis. A person's liability to fall should also be considered [1]. Hormone replacement therapy, bisphosphonates e.g. alendronate, etidronate and risedronate ; , calcitonin, calcitriol and raloxifene are currently used in the prophylaxis and or treatment of osteoporosis [2]. These drugs, which act by decreasing bone resorption by osteoclasts, moderately increase bone density but principally prevent further loss of bone. Evidence that parathyroid hormone PTH ; or its amino-terminal analogues can act as bone-specific anabolic agents, led to the hypothesis that such treatment might be particularly effective in reducing fracture rates in osteoporosis [3]. daily subcutaneous SC ; administration stimulate bone formation but the former result in persistent elevation of PTH serum levels, a consequence of increased bone resorption over time. With daily SC injections, elevations in serum PTH levels are transient and bone formation is favoured [3-5].
The cricopharyngeus is incriminated in swallowing disorders which often involve an asynchrony of pharyngeal constrictors. In cricopharyngeal achalasia there are delays in the opening and closure of the region. Recent work has shown how the vagal contributions are much more significant than the glossopharyngeal components in maintaining the integrity of the swallowing mechanism. Sensory input is dominated by nerve endings within the pillars that encircle the intrapharyngeal opening and receptors within the laryngeal vestibule.

The most important determinant is viral load. Ideally, viral load should be below the limits of quantitation of clinically available assays generally less than 500 HIV-1 RNA copies mL ; . If after 46 months the viral load has not decreased below 500 copies mL, therapy may be continued if other therapeutic options are not available or the initial viral load was high enough that it limited an optimal response. These patients should be monitored closely. An increase in viral load after an initial decline to undetectable levels warrants close observation and consideration of treatment alteration. A viral load greater than 500010 000 HIV-1 RNA copies mL indicates treatment failure and the need to change therapy. Therapy should be altered if there is evidence of intolerance to the medication or of major toxic effects. Immunologic decrease in CD4 count ; , clinical or virologic progression should prompt consideration of a change in therapy and tegretol.

A Schaeffer-Pautz, M.D., N Arora, M.D., K Moles, M.D., M Gadallah, M.D.; Resident, Division of Nephrology Background: Controversy exists among various studies in regard to efficacy of oral PO ; versus parentral calcitriol. Some studies suggest that IV calcijex is superior to PO rocaltrol in treatment of renal osteodystrophy in hemodialysis patients while others suggest their equivalence. In the pediatric peritoneal dialysis, one study showed that after one year, both IP and pulse PO calcitriol equally improved hyperparathyroid bone lesions. Methods: We conducted a prospective randomized study in 32 patients 16 on IP calcijex and 16 on PO rocaltrol ; followed for a period of 48 months. The IP dosing was administered once weekly 2mcg to 6 mcg wk in two divided doses ; . Dose adjustments were made depending on parathroid hormone PTH ; , serum calcium and phosphorus levels. Both IP calcijex and PO rocaltrol were administered in-center under direct supervision of dislysis RN to assure compliance. There was no difference in incidence of peritonitis in both goups. ABSTRACT Identification of the specific muscarinic acetylcholine receptor mAChR ; subtypes mediating stimulation of salivary secretion is of considerable clinical interest. Recent pharmacological and molecular genetic studies have yielded somewhat confusing and partially contradictory results regarding the involvement of individual mAChRs in this activity. In the present study, we re-examined the roles of M1 and M3 mAChRs in muscarinic agonist-mediated stimulation of salivary secretion by using M1 and M3 receptor single-knockout KO ; mice and newly generated M1 M3 receptor double-KO mice. When applied at a low dose 1 mg kg, s.c. ; , the muscarinic agonist pilocarpine showed significantly reduced secretory activity in both M1 and M3 receptor single-KO mice. However, when applied at higher doses, pilocarpine induced only modestly reduced 5 mg kg and duricef.

To explain events due to lowered serum phosphorus one must follow the changes of renal production of calcitrol as they occur. Calcitriol can also inhibit the synthesis of inducible nitric oxide syntheses and increase glutathione levels, suggesting a role for the hormone in brain detoxification pathways and omnicef.

Mia stimulates calcitrkol synthesis via 25-hydroxyvitamin D1 -hydroxylase in the kidney, leading to increased calcium and phosphorus absorption in the intestine and enhanced mobilization of calcium and phosphorus from bone FIGURE 1 ; . The resultant increased serum calcium and increased calcitrill inhibit PTH secretion, with a subsequent increase in urinary calcium excretion and increased tubular reabsorption of phosphorus. Thus, normal serum calcium levels are maintained and serum phosphorus levels are returned to normal. In addition, hypophosphatemia is a potent stimulator of renal tubular reabsorption of phosphate. The kidney is the principal organ that regulates phosphate homeostasis. Serum inorganic phosphorus is filtered by the glomerulus and 80% of the filtered load is reabsorbed predominantly along the proximal nephron. Regulation of proximal renal tubular reabsorption of phosphate is achieved through changes in the activity, number, and intracellular location of the brush border membrane type IIa sodium-phosphate cotransporter NaPiIIa ; . Parathyroid hormone is the bestcharacterized physiological regulator of phosphorus reabsorption, but its principal function is to maintain calcium homeostasis. Parathyroid hormone increases urinary phosphate excretion via cyclic adenosine monophosphate dependent inhibition of NaPiIIa expression. This effect is rapid and is achieved by internalization of NaPiIIa transporters from the brush border membrane and enhanced lysosomal degradation. However, this classic PTHvitamin D axis does not account for all the complexities of phosphate homeostasis, and the study of renal phosphate-wasting syndromes has revealed several novel regulators.

Calcium is a divalent metal essential for the maintenance of the nervous, muscular, and skeletal systems as well as for cell membrane and capillary permeability.

These drugs produce drowsiness, loss of coordination, nausea, unconsciousness and memory loss. In combination with calcitriol, dexamethasone results in a significant time- and dose-dependent increase in vdr protein and an enhanced apoptotic response as compared to calcitriol alone.

Calcitriol and hyperphosphatemia

When used with weekly docetaxel taxotere ; , calcitriol produced encouraging results in a single-center phase ii clinical trial. Vasculature walls.6-8, 24-27 Hyperphosphatemia, hypercalcemia, elevated CaxP, and increased PTH levels are all believed to contribute to this process, although its pathogenesis remains unclear. Calciphylaxis is, in essence, a severe and systemic form of vascular and soft tissue disease. In fact, vascular calcification may represent an independent risk factor for cardiovascular disease in patients with ESRD.5 It likely contributes to the extremely high mortality rate from cardiovascular causes in such patients.6, 27 There is increased risk for ectopic calcification when the CaxP value exceeds 60 to 75 mg2 dL2.7 Specific goals for CaxP have been established to minimize the risk of calcification due to alterations in calcium and phosphorus from CKD itself, and from the effects of pharmacologic therapy. THE PARATHYROID GLAND Anatomic and Physiologic Considerations The parathyroid glands are either embedded within the capsule of the thyroid, or, as their name ie, Gk.: para, alongside ; implies, lay around the thyroid gland. The normal adult parathyroid glands average 40 mg each and have a total combined mass of 160 mg.28 Along with excessive stimulation of release of PTH from parathyroid cells, their mass increases progressively with CKD.8 This increase in mass may follow one of the following patterns: 1 ; diffuse hyperplasia polyclonal ; , 2 ; nodular growth monoclonal ; within diffuse hyperplastic tissue, or 3 ; diffuse monoclonal hyperplasia "adenoma" or tertiary autonomous hyperparathyroidism ; . In hyperplastic states, the size of the glands may reach 10 to 50 times normal.1 Although 80% of normal adults have four parathyroid glands, as few as two or up to twelve may be present.28 Hyperphosphatemia also promotes parathyroid gland hyperplasia. Excessively high phosphate levels may influence post-transcriptional events that increase PTH messenger ribonucleic acid translation and thereby increase PTH synthesis.29, 30 Decreased calcitriol production.
