Introduction: The wireless capsule endoscopy CE ; having been established in our hospital three years ago has shown valuable diagnostic results in patients with obscure gastrointestinal bleeding and in chronic inflammatory bowel disease. Beside to our experience in about 120 adult patients we gained preliminary results in pediatric patients. Methods: Twelve patients 8 f, 4 m ; , years old, were examined with the capsule endoscopy from 2002 to 2004. Indications were diarrhea suspicious of Crohn's disease n 8 ; , the possible extention of proven Crohn's disease into small bowel n 2 ; and two cases with obscure gastrointestinal bleeding. The examinations were performed according to the recommendations for adult volunteers, without any special preparation after fasting period of twelve hours. The data were analysed retrospectively. Results: The average passage time of the capsule through the stomach was 104 minutes, through the small intestine 223 minutes, respectively. No complications related to the examination have been occurred. In most patients 11 12 ; pathological findings have been detected. Seven of eight patients with diarrhea revealed fibrinous ulcerations and circumscript red lesions suspicious of changes related to Crohn's disease. The two patients with proven Crohn's disease of the large bowel showed also pathological findings in the small intestine. In two patients with obscure bleeding a polypoid lesion and an angiektasia were found, respectively. Conclusion: The CE showed in eleven of twelve pediatric patients with Crohn's disease and obscure bleeding a remarkable efficiency in terms of clarification of the diagnosis and the extention of inflammatory mucosal changes of the small bowel. Additionally, this diagnostic method could enable us to treat pediatric patients with more appropriate therapy at an earlier stage.
