Hypercalcemia is virtually always present in patients with primary hyperparathyroidism, while serum phosphorus tends to be in the lower range of normal. In some patients particularly those with mild primary hyperparathyroidism serum calcium may be normal on some measurements. In elderly patients, all calcium concentrations should be `corrected' to the prevailing serum albumin level calcium to be adjusted upwards by 0.8 mg dl for every g dl reduction in serum albumin from the midnormal range of 4.0 g dl ; . The most common other cause of hypercalcemia in older individuals, hypercalcemia of malignancy, is associated with suppressed levels of PTH. Therefore, an elevated level of PTH in the presence of hypercalcemia virtually establishes the diagnosis of primary hyperparathyroidism, although the normal range for the PTH-intact assay generally 10 65 pg does not take into account the fact that PTH levels rise with age. Serum alkaline phosphatase activity may be elevated. Increased bone turnover may be reflected in elevated levels of markers of bone formation bone-specific alkaline phosphatase and osteocalcin ; and bone resorption urinary pyridinoline, deoxypyridinoline and N-telopeptide of collagen ; , even in the absence of skeletal complications 2 ; . However, bone markers increase with age, and it is not clear that elevated levels of bone markers are predictive of the likelihood of bone loss and fractures in older individuals with primary hyperparathyroidism. Urinary calcium excretion may be elevated as well, but this varies widely and is often not increased in elderly patients. While the circulating 1, 25-dihydroxyvitamin D concentration may be elevated, levels of 25-hydroxyvitamin D tend to be low or in the lower range of normal in older individuals. In most older patients, no specific radiological manifestations are observed, and skeletal x-ray screening is not recommended. However, bone-density measurements based on dual-energy x-ray absorptiometry DXA ; at the hip and spine should be an integral part of the evaluation of individuals with primary hyperparathyroidism, regardless of age. As indicated, for instance, esomeprazole mechanism of action.

Patients with acute basilar artery occlusion have a mortality rate greater than 85% and most survivors have severe persisting disability. Advances in thrombolysis and endovascular therapy may increase the survival rate and limit the disability rate. We present a case of a 70 years old male, with history of dislipidemia, hypertension and atrial fibrillation that was admitted to our hospital in November of 2005 due to rapid onset of vertigo and vomiting followed by decreased level of consciousness, internuclear ophthalmoplegia and right hemiparesis. Cerebral angiography was performed showing basilar artery stroke, and he was treated with basilar intra-arterial thrombolysis and a right vertebral artery stent was placed. he was transferred from Neurology to our PRM Department, where he was submitted to a rehabilitation program, with functional improvement, traduced by Modified Barthel Index 2 at admission and 55 at discharge ; and in Functional Independence Measure 62 at admission and 78 at discharge ; . This case illustrates the importance, in selected cases, of this therapeutic option, indicating that good functional outcome could be expected in patients treated with intra-arterial thrombolysis. More randomized controlled trials are needed to establish the efficacy of this therapeutic modality and estrace.
In 2005 the Company delivered excellent results, substantially ahead of market expectations at the beginning of the year as strong sales growth was enhanced by productivity gains to yield very strong earnings growth. This was especially gratifying given the challenges and uncertainty we faced following some disappointments in 2004. AstraZeneca was put to the test in 2005 and these results show how well we responded. Such an experience will prove of great value in preparing the Company to face new challenges in the future. AstraZeneca's strength derives from its outstanding portfolio of products, its global reach and, above all, the creativity and commitment of its employees. Our marketed product range continues to develop in both strength and depth. AstraZeneca now has ten products each with global sales of over $1 billion. Several of these, products such as Nexium, Seroquel, Crestor, Arimidex and Symbicort, are still enjoying very strong sales evolution and will continue to be the engines for growth in the medium term. Nexium achieved sales of $4.6 billion in 2005 benefiting from good clinical differentiation and strong branding. In this large and highly competitive market, it was no surprise when we were notified that a manufacturer of generic drugs, Ranbaxy Laboratories Limited, had submitted an Abbreviated New Drug Application ANDA ; for esomeprazole magnesium the active ingredient in Nexium ; in the US. We have full confidence in our intellectual property, which we will continue to defend vigorously and we have filed a lawsuit in the US District Court of New Jersey against Ranbaxy Laboratories for wilful patent infringement. Seroquel, with $2.8 billion sales in 2005, further strengthened its position as the most prescribed atypical anti-psychotic therapy in the US and continued to grow strongly in other markets. A second phase 3 clinical trial has confirmed earlier results and enabled a supplemental.

Organiser: 3E, 1, rue du Parc, 92593 Levallois-Perret Cedex. Tel: + 33 1 Fax: + 33 1 According to government figures, total pharmaceutical spending in 2000 amounted to 25 billion euros US$23 billion ; , an increase of 9.3% over 1999. This equates to per capita spending of 415 euros US$382 ; , or 1.1 euros US$1 ; per person per day, one of the highest levels in the world. Between 1990 and 2000, pharmaceutical spending increased in value by 73.4%, representing a compound annual growth rate of around 5.7%. In recent years, pharmaceutical expenditure growth has followed a stop-start cycle as a result of various government intervention measures. Thus in 1994, growth was less than 3% due to the introduction of cost containment programmes such as the implementation of medical monitoring, the signing of the state-industry framework "convention" agreement, as well as a low incidence of communicable disease. Growth in 1995 accelerated to over 6%, partly due to the government's concerted attack on hepatitis B that year, whilst the Jupp reform programme and the threat of economic sanctions against doctors who overprescribe held growth down to between 3%-4% in 1996 and 1997. In 1998, growth accelerated again to over 6%, and grew at a similar rate in 1999, before rising to 9.2% in 2000. The launch of costly new drugs is a prime factor in recent spending rises. Whilst pharmaceutical expenditure continues its upward trend, drug consumption, measured in terms of quantity, has stagnated at around 3 billion units per annum since 1995, although preliminary figures for 2000 indicate a slight rise. This rise is attributed to the universal health charge reforms, which have improved access to medicines for those sections of society not previously covered by the social security system. Market volume is split roughly evenly between prescription only medicines and OTC products. The French market for medications was valued at 20.4 billion euros at retail prices in 1999 US$21.8 billion ; by the pharmaceutical research agency GERS. By value, prescription only medicines represented 70% of the market. The remaining 30% comprised OTC products, that is medications which can be obtained without a prescription, although two-thirds of these products were in fact obtained on prescription, because they are eligible for reimbursement under the social security scheme. According to the pharmaceutical industry association, SNIP, sales of drugs on the French market amounted to 18, 675 million euros at manufacturers' prices in 2001, an 8.2% increase over the 17, 263 million euros recorded in 2000. In 2000, sales were split 85.1% retail pharmacy sales with reimbursable products accounting for around 92.3% of the total ; and 14.9% hospital sales. In 1999, products aged 10 years or more continued to account for nearly a half of sales in the retail pharmacy market, despite the fact that new presentations of old products are classed as new products from the launch date of the new presentation. The 50 best selling products accounted for 26% of the retail pharmacy market by both value and quantity in 1999, whilst the top 100 represented 40% of sales by value and 38% by quantity, and the top 500 accounted for 81% by value and 75% by quantity. The top five therapeutic groups account for around three-quarters of sales, both in terms of volume and value. Cardiovasculars generate the most sales in terms of revenue with a 26.2% share of the market in 1999, but lower-priced CNS drugs are the most frequently consumed products with 23.9% of the market by volume. Analgesics account for around 60% of CNS drug sales by volume, and 38% by value, whilst antibiotics account for nearly 80% of sales of anti-infectives by volume and 70% by value and glibenclamide.
