Most subjects were referred by gynecologists for chronic vulvar symptoms that were unresponsive to conventional treatments. The subjects' medical records were then reviewed to determine if they met the inclusion criteria for the study. Inclusion criteria consisted of a diagnosis of vulvovaginal lichen planus, which was confirmed by biopsy, physical exam, and symptoms. Data were collected from review of the charts and included the following variables: gravidity, parity, age in years at presentation, ethnicity, pretreatment yeast culture, pre- and posttreatment symptoms, pre- and posttreatment examination findings, duration of symptoms, presence of oral or skin lesions, and duration of treatment. All subjects in this practice with the diagnosis of vulvovaginal lichen planus were treated with hydrocortisone 25 mg suppositories 1 21 ; twice a day intravaginally and then slowly tapered off to a symptom-free maintenance dose eg, once or twice weekly ; . If their symptoms recurred they were restarted on hydrocortisone 25-mg intravaginal suppositories twice a day and again tapered to the lowest possible dose for maintenance. Response to treatment was measured clinically by preand posttreatment observation of erythema, erosion, stenosis, and resolution of the lesions. Response to treatment subjectively was measured by pre- and posttreatment patient self-report of burning, discharge, dyspareunia, apareunia, and pruritis. Clinical and subjective responses were evaluated at the time of initial follow-up after starting treatment. An overall impression of response clinically and subjectively was assigned by the investigators as worse, unchanged, some improvement, or complete improvement. All examinations were performed by a single physician. Data were analyzed using SPSS Statistical Package for the Social Sciences; SPSS Inc., Chicago, IL ; software package. McNemar 2 was performed to detect if there were significant changes in subject responses to treatment. Significance was set at .05. RESULTS Sixty subjects with the diagnosis of vulvovaginal lichen planus were identified. Subject ages' at presentation ranged from 30 84 years with a mean age of 58 13.2. The mean gravidity of subjects was 3.0 2.1 with a range of 0 10 pregnancies. The mean parity was 2.5.
