There are currently two classes of medications that have demonstrated efficacy in managing moderate to severe AD: the NMDA noncompetitive antagonist, memantine, and the cholinesterase inhibitors, donepezil, galantamine and rivastigmine. Studies have identified glutamate as being critical in the formation of memory as well as synaptogenesis. Memantine's utility in AD emerges from its ability to inhibit pathologic stimulation of NMDA receptors while allowing physiologic stimulation. This duality can potentially lessen the effects of excessive glutamatergic stimulation and calcium damage to neurons in the neurodegenerative pathway of AD, while preserving cognitive functioning. Acetylcholine is also an excitatory neurotransmitter that is important in cognition and memory. There is well-documented cholinergic neuronal degeneration in AD, particularly in the more severe stages. The acetylcholinesterase.

Effect on the cholinergic system makes galantamine a promising agent for AD. Comparative trials with other acetylcholinesterase inhibitors are lacking. Efficacy Galnatamine has been evaluated in large n 285 to 978 ; , well designed trials of 3 to months' duration in patients with mild to moderate AD, as well as in several small nonblinded studies. Gxlantamine 16 or 24 mg day demonstrated efficacy at 3 to months across all studies, with significant differences from placebo observed for all primary and most secondary efficacy assessment measures. Galantam9ne treated patients showed significant improvements in cognition, behavioral symptoms and activities of daily living compared with placebo recipients. Activities of daily living outcomes were significantly better in these galantamine treated patients than placebo recipients. Long term evaluation indicated that galantamine 24 mg day maintained cognition and activities of daily living in patients who had received this dose throughout a 12 month study period 6 months' double-blind, then a 6-month extension ; .4 Side Effects Galantamije was well tolerated in patients with AD. Side effects were those expected from an AchE inhibitor. Overall, 19% 441 2287 ; of patients treated with galantamine, were discontinued from Phase III controlled clinical trials due to side effects compared to 8% 98 1159 ; in the placebo group. For patients treated with galantamine, the rate of discontinuation due to side effects was 14% for males and 22% for females. The most common.

There has been concern that interictal spikes, particularly rolandic spikes, with or without clinical seizures, can be associated with attention deficit hyperactivity disorder and deficits in visual perception, short-term memory, and overall cognitive functioning Deonna et al, 2000; Holtmann et al, 2003; Weglage et al, 1997 ; . In children with benign epilepsy with centrotemporal spikes BECTS ; , LEV has been shown to reduce seizure frequency and cause normalization of electroencephalography EEG ; Bello-Espinosa and Roberts, 2003 ; . LEV is a novel antiepileptic drug that is indicated for use as adjunctive treatment of partial onset seizures in adults and children 4 years of age and older with epilepsy. Thus, we hypothesized that children with attentional learning difficulties and "subclinical" spikes would have improvement of neuropsychological functioning if treated with LEV, for example, galantamine 16 mg.

Donepezil, galantamine and rivastigmine are AChE inhibitors that were reviewed by NICE in January 2001 and recommended for patients with mild and moderate AD, with continued treatment being dependent on the patient's showing a response to the drug. This is a review of the original appraisal, and also a new appraisal of the clinical and cost effectiveness of memantine. Memantine is not an AChE inhibitor it works in a different way and was not recommended as a treatment option except as part of well designed clinical studies; therefore, the costs of this have not been considered. Following review of more recent evidence the use of AChE inhibitors is now recommended as options in the management of people with AD of moderate severity only. People with mild AD who are currently receiving donepezil, galantamine or rivastigmine may be continued on therapy until it is considered appropriate to stop and ketoconazole. A: All medications can have side effects. Not all medications cause side effects in all people. Medications can cause side effects in some people, for example, galantamine 4 mg. In my experience with patients, a baby's low birth weight--as well as many of the other problems cited above, whether or not the birth weight was low--can most often be linked to an infected uterine environment. This fact adds to my conviction that the first nine or fewer ; months that we live in the uterus have a far more significant effect on everything we become as human beings than all of the years we live after birth. In cases of conception and birth after the uterine environment has been restored to health by comprehensive antibiotic therapy, the results have almost always been unilaterally positive: an uncomplicated pregnancy, a trouble-free delivery within a few days of the projected due date, and, most gratifying of all, a normal-weight baby who exhibits none of the abnormal physical, mental, or emotional problems cited above. When I first interview people who come to my clinic, anything short of this positive scenario in their prior reproductive history sends up a warning signal of possible infectionrelated infertility. The most critical reason to expand our understanding of what constitutes infertility is to allow for the earliest and therefore most effective possible treatment. Unfortunately, in cases of reproductive difficulty where pathogens are involved--and I believe they are implicated in the majority of situations--a sad truth prevails: the longer the condition exists, the stronger it grows and the more likely it is to leave behind irreparable damage. For infertile couples whose infections go untreated, this translates into an ever-escalating risk that they won't be able to conceive, that the prospective mothers will suffer complications during pregnancy, or that the health of the baby assuming birth occurs ; will be compromised and lamisil. 2006; nice TA111 16. Two monkeys vomited within 30 minutes after administration of d-amphetamine in combination with 1.0 mg kg galantamine. At the two lower doses no emetic events were observed. See also Table 2 and levofloxacin and galantamine. The scientific basis for recommending donepezil, rivastigmine, or galantamine as preferred treatment for patients with Alzheimer's disease is questionable because minimal benefits were measured on rating scales and the methodological quality of the available trials was poor. Nineteen out of 22 randomised controlled trials evaluating the efficacy of donepezil, rivastigmine, and galantamine show significant differences between treatment groups and placebo, indicating a beneficial effect of cholinesterase inhibitors, but the differences are rather moderate. The gains of 1.5-3.9 points in cognitive function, as measured with the Alzheimer's disease assessment scale, fall below the 4 points that a panel of experts from the US Food and Drug Administration proposed as the minimum of a clinically important effect.13 However, the assumption that 4 points on the scale is clinically relevant is an expert opinion and presumably not evidence based. The results on the clinician's interview based impression of change with caregiver input scale, are moderate as well. The reported differences of 0.26-0.54 points are smaller than the allowed variation for one patient who can only get full scores. Also, the retest reliability of the scale is reported to range from 0.4 points to 0.6 points.14 Owing to several methodological shortcomings, the validity of the reported small findings seems to be limited. Missing rigour might have led to an overestimation of beneficial effects. On the other hand, the trials provide clear evidence for considerable adverse events. Comparison with other studies The results of our review are in contrast to numerous publications that support the use of cholinesterase inhibitors. The reason for this discrepancy can be explained by differences in assessment criteria for the methodological quality of the trials. The Cochrane systematic review on donepezil15 reports clinical efficacy of donepezil despite reporting the fact that only three of the included studies describe the method of randomisation in sufficient detail and that dropout.

Have to be considered before it is used in a given patient. For example, donepezil is 100% absorbed and is not associated with any food interactions. It has a half-life of 70 hours, which makes it a true once-daily medication. Also, because of its long half-life, no discontinuation-type symptoms are expected if the drug is stopped abruptly.10 All of these are important points, especially in this elderly population who may forget a dose and who may not remember to take medications more frequently than once per day. Galantamine and rivastigmine both have shorter half-lives and therefore need to be dosed twice per day.11, 12 Donepezil and galantamine are metabolized by CYP 3A4 and CYP 2D6.9, 13 This means that inhibitors of these enzymes could theoretically increase the blood levels of these medications. The common inhibitors of. Table 3. Total and segmental pulmonary vascular resistances pre- and post-U-46619 constriction in the presence of NOS inhibition with L-NNA. The present study demonstrates that galantamine facilitates both excitatory and inhibitory transmissions in the rat and human brains and that these effects are the result of the nicotinic APL action of galantamine. The concentrations at which galantamine facilitates synaptic transmission are very similar to those achieved in the brain of rats treated with memory-enhancing doses of the drug Bores et al., 1996 ; . Thus, facilitation of synaptic transmission by galantamine is likely to underlie its therapeutic effectiveness in AD.
