Methods: The national Discharge Registry in Finland has served as a basis for the collection of all the gynaecological procedures performed in Finnish hospitals since 1987. The annual variation of different major procedures in gynaecology and their follow-up from 1987 - 2004 was analyzed to evaluate the trends in these procedures. Results: Hysterectomy has been the most prevalent procedure in women in Finland in 1980's and 1990's. This gynaecological index operation showed a low increasing trend until 1998 and a subsequent decrease until the present. Since the early 1990's the distribution of different types of hysterectomies showed an increase in laparoscopic approach as well as in vaginal hysterectomies. The abdominal i.e. open ; hysterectomies have decreased in numbers under that of vaginal ones and are rather similar to the number of laparoscopic hysterectomies in 2004. Previously popular supravaginal amputations have almost disappeared and in 2004 only 194 2.1% ; supravaginal procedures mostly elective ; out of 9234 hysterectomies were performed. Another dramatic change in surgical procedures in gynaecology has happened in incontinence surgery. The old golden standard, the Burch procedure, has been replaced intially by TVT in late 1990's and more recently by TOT. Conclusions: The evaluation of the trends of gynaecological surgery recalls a critical and regional analysis of the outcome: on one hand to the morbidity and on the other hand to the politics and economics related to these procedures. Consequently, these data can be used to assist the development of national health care services in gynecology in Finland, because soy isoflavones ttc.

More than 70 years of scientific inquiry surround the relationship between soybean intake and thyroid function. Recent concerns include the possible goitrogenic effects of soybean isoflavones. As increasing numbers of consumers embrace soyfoods, health professionals need to understand both the benefits and the potential challenges of soy consumption. Introduction The thyroid gland is located in the lower part of the neck wrapped around the front of the trachea. The thyroid gland produces three hormones: calcitonin, which is involved in calcium regulation; and 3, 5, 3'-triiodothyroine T3 ; and 3, 5, 3', T4, thyroxine ; , commonly referred to as the thyroid hormones. Iodine is an essential component of both T4 and T3, contributing 65 percent and 59 percent of their molecular weights, respectively. The normal thyroid weighs between 15 and 20 grams. However, in response to prolonged iodine deficiency, the thyroid gland can increase about fivefold to the size of a football, a condition recognized as goiter. The primary function of the thyroid gland is to produce thyroid hormones, which primarily function to regulate basic metabolic rate. When serum iodine levels are low or when the thyroid doesn't receive enough iodine, the anterior pituitary secretes more of a hormone called thyroid stimulating hormone TSH ; . This excessive secretion of TSH compensates by increasing the size of the thyroid and thereby allowing it to work more efficiently. The primary cause of goiter and hypothyroidism around the world is inadequate iodine intake. Approximately one billion people are considered to be iodine deficient worldwide.1 The adult iodine RDA is 150 ug, so one teaspoon about 4-5 g ; of iodine is all a person requires in a lifetime. Yet, iodine deficiency is an immense problem in many regions in the world, especially in the Himalayas, the Andes, and in the central part of Africa. Fortunately, approximately 50 percent of the U.S. population uses iodized salt which is why the iodine intake of the U.S. population is considered adequate. Some subsets of the population, such as women of child-bearing age, still have marginal iodine intakes, however, 2, 3, 4 and thyroid problems can also develop in populations consuming adequate amounts of iodine. Thyroid abnormalities in these situations are often due to the presence of dietary goitrogens, such as glucosinolates in cruciferous vegetables, which interfere with utilization of iodine or functioning of the thyroid gland. Soy Early Research The first report that soybeans produced goiter in iodine-deficient rodents was published in 1933.5 Subsequent animal studies published in the 1930s and 1940s noted similar findings.6, 7, 8 Iodine fortification of the diet largely eliminated this problem, although soyfed animals are known to require.
We observed no such specificity. Furthermore, the isoflavones had no effect on the mRNA abundances of ACAT1 or ACAT2. This contrasts our recent findings that the citrus flavonoids, naringenin and hesperetin selectively decreased the expression of ACAT2 [36]. Genistein is a known tyrosine kinase inhibitor with an IC50 of 20 M, whereas daidzein does not inhibit tyrosine kinases at the concentrations used in the present study [48]. Despite this, daidzein, like genistein, inhibited apoB secretion, cholesterol synthesis, ACAT activity, MTP activity and expression as well as enhancing LDL receptor expression. Therefore, it is unlikely that the ability of genistein to inhibit tyrosine kinases is solely responsible for the ability of this isoflavone to inhibit apoB secretion. The isoflavone-induced increases in LDL receptor mRNA may be due to the estrogenic activity of these compounds. Genistein and daidzein are weak estrogenic compounds with 8% and 3% of estradiol activity, respectively [49]. Estrogen has been shown to increase hepatic LDL receptor expression in animal models [32] and increase LDL receptor transcription in HepG2 cells [33]. On the other hand, it is unlikely that the estrogenic action of these isoflavones is directly responsible for their effects on apoB secretion and cholesterol esterification in HepG2 cells observed in the present study. Estrogen has been reported to cause no change [50] or even increase [51] apoB secretion from HepG2 cells. Furthermore, estrogen treatment results in no change [52] or an increase [53] in hepatic ACAT activity. Our results are consistent with studies conducted in animals and humans [12-18, 41], showing reduced plasma cholesterol levels following administration of diets containing soy protein rich in isoflavones. However, the contribution of the isoflavones themselves to the modulation of plasma lipids has not been clearly established. Several human studies demonstrated no lipid-lowering effects of isolated isoflavones [54-56]. Furthermore, diets supplemented with an isoflavone extract from red clover, enriched in genistein and daidzein, did not result in any changes in plasma lipids [reviewed in 41]. It is possible that the bioavailability of the isoflavones is limited when provided in extract form. However, Setchell et al. recently demonstrated that the Cmax values in plasma, following single 50 mg oral doses of either genistein or daidzein to healthy young women, were 1.26 0.27 and 0.76 0.12 M, respectively [57]. Since isoflavones are subject to extensive first-pass clearance by the liver, it is feasible that human hepatocytes are exposed to isoflavone concentrations similar to those used in the present study. However, the possibility remains that another component of soy is required for the isoflavones to exert their effects on plasma lipid concentrations. Differences in the systemic bioavailabilities of genistein and daidzein [57] may also be reflected in our cell culture studies. We cannot rule out the possibility that any differences in the extent of the effects we observe are due to differences in the uptake and metabolism of genistein and daidzein in HepG2 cells. In conclusion, the soy phytoestrogens, genistein and daidzein, significantly reduced the hepatic secretion of apoB-containing lipoproteins, an effect mediated by reductions in cholesterol synthesis, cholesterol esterification and MTP activity and enhanced expression of LDL receptors. Many forms of hypercholesterolemia are characterized by hepatic overproduction of apoB-containing lipoproteins [58]. Our results demonstrate that compounds such as genistein and daidzein may prove to be useful for treatment of these lipoprotein disorders. Furthermore, our findings suggest a new therapeutic paradigm and levothyroxine.
For two years SoyaWorld, a soy milk and soy products company, has run newspaper ads for the "Run for the Cure" promotion. They also distribute coupons for So Good products with 25 cents from each coupon donated to the CBCF. SoyaWorld produces both organic and non-organic soy products. The So Good product associated with the CBCF is a non-organic soy product. Although soy beans have possible cancer-fighting phytochemicals including isoflavones, they are also among the most heavily sprayed crops in the U.S. and Canada.30 For this reason, Soyaworld was asked if they knew what pesticides were used on the non-organic soy beans used in their products, among the usual questions asked for this research project. SoyaWorld returned the inquiries with the above information and did not answer the pesticide question. Notably, the returned the email contained a copy of the original questions but the one concerning pesticides was not only not answered, it had been deleted and lithobid.

Methods: A retrospective case notes review was undertaken of all cases of T. vaginalis infection diagnosed in females in a 12 month period between January 2003 and December 2003 n 155 ; . Descriptive features of these patients were collated. Results: The incidence of T. vaginalis infection was 3.0% in 2003. The mean age of patients was 28.8 years. Black Caribbean and Black African women were over-represented. Overall, patients were more likely to be symptomatic at presentation 78.1% ; and have vaginal discharge as their presenting symptom 87% ; . 19% had co-existing chlamydia infection and 7% had co-existing gonorrhoea infection. There were 2 new HIV diagnoses. Culture improved diagnosis, identifying an additional 10% of cases. Contact tracing was initiated in 81.3% of cases. There was one `true' treatment failure. Conclusions: The mean age of 28.8 years is lower than that quoted in other studies. Most patients were symptomatic at presentation. The rate of co-infection with chlamydia was high, we should consider giving empirical treatment for chlamydia in patients diagnosed with T. vaginalis infection. Routine test of cure could be stopped.
