Systematic targeted deletion of individual isoforms has identified at least some of the variants that mediate catabolic effects. At Green Lane Hospital patients screened by a cardiology consultant or registrar have their cardiac exercise tests supervised by a cardiac technologist, with the consultant registrar immediately available. Clinical and procedural data were collected in a spreadsheet. Results There were 379 patients with adequate data, aged 2589 years, mean 58 years, 45% female, 55% male. Known coronary artery disease CAD ; was present in 117 patients Group 1 ; of whom 39% had a positive test 1 mm ST change ; compared with 20% with a positive test in the 262 patients without CAD Group 2 ; . In Group 2, 33% of the patients with a negative test failed to reach their target heart rate. In Groups 1 and 2, 53% and 21% respectively were on beta-blocking therapy. Seven and four per cent of patients required glyceryl trinitrate spray and transient oxygen therapy respectively, and one was admitted to CCU after prolonged ST segment depression without infarction. Two patients had non-sustained ventricular tachycardia 4 and 6 beats respectively ; , one patient each had second-degree atrioventricular block and controlled atrial flutter.

The complexity of these situations arises from the fact that one is looking at a trade-off between efficacy and other issues regarding side-effects, patient acceptability and costs. Although such trials are often presented as noninferiority trials, it is possible that some modest reduction in efficacy may be acceptable alongside the other benefits of a new treatment. One such example of this may be the acellular pertussis vaccines, which are used in preference to whole cell pertussis vaccines because of fewer adverse events, although their efficacy may be somewhat lower. Another issue is whether any particular trial comparing a new treatment with an active standard treatment should be formulated as a noninferiority trial or not. Given the relative state of ignorance with which one starts any new study, one is often unsure whether to optimistically pursue the prospect of demonstrating a new treatment's superiority or whether to settle for demonstrating noninferiority on the basis that, that it will be still good enough to make the new treatment of some value.
