This drug is prescribed to control symptoms of parkinson's disease. Taking this.'" In fact, in this issue of the Journal, there is a report of a long-term follow-up analysis that found that people in the CARET trial may still be at an elevated risk of lung cancer some 6 years after discontinuing the supplements see p. 1743 and related editorial, p. 1729 ; . One hopeful discovery came from the ATBC, though--a one-third reduction in prostate cancer incidence among vitamin E users. Albanes said that prostate cancer was not the cancer that most researchers expected to be affected by the antioxidants. "If confirmed by SELECT [Selenium and Vitamin E Cancer Prevention Trial], it will have tremendous public health impact, " Albanes added. Tamoxifen, an estrogen antagonist, has a long history in medical research. It was originally studied as a form of birth control, but it increased fertility. Then it moved into breast cancer studies, first as an adjuvant treatment for women with advanced breast cancer. New studies expanded its applicability to early-stage disease. When tamoxifen was first studied as a chemopreventive agent, the potential side effects of uterine cancer and blood clots were already known. That didn't stop the studies. Women at high risk for breast cancer because of family or personal history were enrolled, and the FDA approved tamoxifen as the first chemopreventive agent for breast cancer in 1998. A breast cancer risk assessment tool was developed by NCI, and tamoxifen's packaging contains a black box warning stating the risks of death from strokes, pulmonary emboli, and uterine cancer. A study published in JNCI in 2003 stated that 2 million women at high risk for breast cancer could benefit from tamoxifen use without undue risk. Risk Is Relative "There is no such thing as a healthy person, " said Michael B. Sporn, M.D., in the Department of Pharmacology and Toxicology at Dartmouth Medical School. "We are all at some risk. The higher the risk, the more willing one is to put somebody on a preventive agent that might have some risk associated [with it]. The more information about chronic safety of a drug, the better off we are in terms of trying to use it in chemoprevention." Waun Ki Hong, M.D., at the University of Texas M. D. Anderson Cancer Center in Houston, takes this idea a step further. For some patients, the risk is very high; therefore, he regards some of today's chemoprevention as early cancer treatment. Oral leukoplakia, for example, is an aggressive manifestation that can often lead to cancer. The question is how to treat those lesions that will in all Dr. Michael Sporn likelihood progress to cancer. Hong is testing Celebrex in patients with these lesions, and in other precancerous conditions as well. He says he is also talking with the FDA and Astra Zeneca about testing their lung cancer therapy drug, Iressa gefitinib ; , an epidermal growth factor receptor EGFR ; inhibitor, as a lung cancer prevention agent in the near future. Prevention researchers are already thinking about how to fill the chemoprevention pipeline. "Right now, most drugs being used for cancer prevention were initially developed for completely different purposes, " said DuBois. "Nobody has a large, wellfunded program under way to develop primary cancer chemoprevention agents." That needs to change, said Wickerham. "That's a long-term activity, but it makes more sense. I've heard that several drug companies have begun exploring possibilities in that arena." Extending patent protection for drug makers to test their drugs as chemoprevention agents would help move drugs into the investigational pipeline, Wickerham added. Today, patents last 17 years. By the time an agent is tested and approved for one indication, then moved into chemoprevention for years of additional testing, the patent that ensures market share and future profit is close to expiring. Different Strategies Sporn said intermittent drug administration might reduce side effect risks. He has tested a rat model of breast cancer, giving a chemopreventive agent for one month, followed by several weeks without chemotherapy, then more treatment. "In light of the Vioxz result, which gives us a warning that a drug that is relatively safe over the short term may not be so safe when given chronically, intermittent administration needs to be looked at in chemoprevention, " Sporn said. He is a fan of combination chemoprevention as well. Drug combination "is a very important path to follow, " agreed NCI's Hawk. "Based on data in animal systems, it typically allows one to decrease the doses of the individual drugs, yet retain a more potent effect." NCI has two combination chemoprevention clinical trials. The first combines sulindac and eflornithine for patients at moderate risk for colon cancer. The second is in individuals with familial adenomatous polyposis, comparing celecoxib alone to celecoxib plus eflornithine. At least one of the pharmaceutical companies in the spotlight is still behind cancer prevention. "From Pfizer's point of view, chemoprevention of cancer is something we still see as a very important area of research, " said Craig Eagle, M.D., Pfizer's medical director of global oncology. "The challenge is finding drugs that do provide a degree of benefit without any problems." Time will tell if others share his view. "If drug companies steer clear of chemoprevention, I fear that this field will have to rely on nutritional compounds, dietary supplements, and other lifestyle alterations alone, " Hawk said. "[The Gioxx withdrawal] could have a cooling effect. It's the last thing I want to see happen, given the opportunities to benefit people at significant risk for cancer." --Cori Vanchieri.

On September 30th 2004, Merck announced the withdrawal of the Cox-II selective inhibitor rofecoxib, based on 3-year data from the Adenomatous Polyp Prevention on Vioxd APPROVe ; trial. This showed a doubling of the risk of confirmed cardiovascular CV ; events in patients taking rofecoxib compared with placebo 1.48 vs. 0.75 CV events per 100 patient-years [relative risk 1.96, 95% CI 1.203.19, P 0.007] ; .1 This increased risk was evident after about 18 months. Rofecoxib obtained its product licence largely on the basis of the VIGOR study, which had a median follow-up of 9 months see MeReC Briefing Issue No. 20 ; . The Committee for Medicinal Products for Human Use CHMP ; recently reviewed the Cox-II selective inhibitors -- celecoxib, rofecoxib, etoricoxibq, parecoxibq and valdecoxibq see MeReC Extra Issue No. 14 ; . The review did not find significant and consistent gastrointestinal GI ; benefits of Cox-II selective inhibitors compared with conventional NSAIDs, but did find a trend towards higher overall CV risk. The question of whether the CV risk of rofecoxib is a class effect remains. In response to the APPROVe study, the CHMP is again reviewing the Cox-II selective inhibitors, including lumiracoxib not currently marketed in the UK ; , looking at all aspects of CV safety. What alternatives to rofecoxib can be considered? Consider whether non-NSAID therapeutic strategies are appropriate e.g. non-drug interventions, paracetamol, topical rubefacients ; . If a NSAID is necessary, prescribe one associated with a low risk of serious upper GI toxicity e.g. ibuprofen ; , at its lowest recommended dose and for the shortest possible period. Always prescribe in accordance with the Summary of Product Characteristics and review regularly. In patients at high-risk of a GI bleed, a conventional NSAID plus a gastroprotective agent e.g. misoprostol ; may be appropriate. However, currently, NICE advises that there is no evidence to support the use of additional gastroprotection with a Cox-II selective inhibitor.2 and xalatan. Introduction The resumption of food intake after fasting is termed "refeeding." Previous studies have established that energy balance during refeeding differs from both fed and fasting states. An increase in "food efficiency" energy accumulated as body mass energy consumed ; occurs during refeeding, with increased rates of protein, lipid, and glycogen synthesis 1-4 ; . We demonstrated increases in selected intestinal digestive and metabolic enzyme activities to levels above those found in fed control animals 2 ; . The adaptations that occur during refeeding are of survival value to the organism since they aid in the recovery from illness, injury, or starvation. These same adaptations also may play a role in the high rate of relapse noted after hypocaloric therapy of obesity 5 ; . The aim of this study was to examine the effect of inhibiting carbohydrate digestion and absorption on the repletion of body weight and recovery of intestinal function during refeeding in the rat. Other studies have investigated induced intestinal nutrient malabsorption and its effects on body weight and serum lipids, eg, using sucrose polyester and zestril.

April 7, 2005 --The FDA has asked the drug company Pfizer to withdraw the arthritis drug Bextra from the market because it says the risks of the drug outweigh the benefits. Pfizer has agreed to suspend sales and marketing of Bextra in the U.S. But in a news release, the company says "it respectfully disagrees with the FDA's position regarding the overall risk benefit profile of Bextra." Pfizer says it will talk with the FDA about ways to let the company restore Bextra's availability to doctors and patients. But for now, Pfizer advises patients to "stop taking Bextra and contact their physicians about other appropriate treatment options." Bextra is an anti-inflammatory drug similar to Vvioxx and Celebrex. Vioxd was removed from the market in September 2004 because of increased risks of heart attack and stroke. Celebrex remains on the market, but some research points to heart risks from it, too. In December, the FDA issued a new warning for Bextra, cautioning that the drug should not be used in patients undergoing heart bypass surgery because a study showed an increase in heart problems in patients taking Bextra after this procedure. The FDA had also strengthened warnings about reports of potentially fatal skin reactions to the drug. A few people have died after having such reactions, which most often occur in the first two weeks of treatment. Celebrex and All Anti-Inflammatory Drugs Affected Bextra isn't the only drug of its class addressed by the FDA today. The agency is also asking Pfizer to include a boxed warning on the Celebrex label. The drug company has agreed to work with the FDA on that. The warning highlights the potential for increased risk of heart attack and stroke with Celebrex as well as the potential for serious, life-threatening stomach bleeding. The FDA has also asked makers of all nonsteroidal anti-inflammatory drugs NSAIDs ; , both prescription and nonprescription, to revise their labels to include more specific information on potential risks of taking these drugs. Bextra and Celebrex are NSAIDs, as are many other dugs, such as ibuprofen and naproxen. Prescription NSAID Changes Based on the available data, the FDA says it will request the manufacturers of all prescription drugs containing NSAIDs to revise their labels to include: A boxed warning regarding the potential for serious or life-threatening risks to the heart and stomach. A warning against use in patients who have recently undergone heart bypass surgery. A medication guide for patients to help make them aware of the potential risks associated with the use of this class of drugs. The guide will inform patients of the need to discuss with their doctor the risks and benefits of using NSAIDs and the importance of using the lowest effective dose for the shortest duration possible. Over-the-Counter NSAIDs The available data do not appear to suggest an increased risk of serious heart events for the short-term, low-dose use of the NSAIDs available over the counter. The FDA says it will request changes to these labels to better inform consumers regarding the safe use of these products. The FDA says it will ask the manufacturers of all nonprescription products containing ibuprofen Motrin, Advil, Ibu-Tab 200, Medipren, Cap-Profen, Tab-Profen, Profen, Ibuprohm ; , naproxen Aleve ; , and ketoprofen Orudis, Actron ; to revise their labels to include and ziac. However, intangibles remain undervalued by capital markets because of insufficient information. When information on IP value is insufficient, securitization deals have stalled. While no information may lead to under-valuation, it must still be established that information when provided leads to higher valuation. It has been demonstrated that more disclosure in IPO prospectuses for pharmaceutical and biotechnology companies correlates with lower stock volatility. It is clear that once investors have information they.

In response to environmental stress, eukaryotic cells reprogram their translational machinery to allow the selective expression of proteins required for viability in the face of changing conditions. During stress, mRNAs encoding constitutively expressed "housekeeping" proteins are redirected from polysomes to discrete cytoplasmic foci known as stress granules SGs ; , a process that is synchronous with stress-induced translational arrest Anderson and Kedersha, 2002; Kedersha and Anderson, 2002 ; . Both SG assembly Kedersha et al., 1999 ; and translational arrest Krishnamoorthy et al., 2001 ; are initiated by the phosphorylation of translation initiation factor eIF2 , which reduces the availability of the eIF2GTPtRNAMet ternary complex that is needed to initiate protein translation. Drugs that stabilize polysomes e.g., emetine ; cause SG disassembly, whereas drugs that dismantle polysomes e.g., puromycin ; promote the assembly of SGs, indicating that mRNA moves between polysomes and SGs Kedersha et al., 2000 ; . These results suggest that SGs are sites of mRNA triage at which mRNP complexes are monitored for integrity and composition and are then routed to sites of reinitiation, degradation, or storage Anderson and Kedersha, 2002; Kedersha and Anderson, 2002 ; . During stress, mRNA continues to be directed to sites of reinitiation, but in the absence of eIF2GTPtRNAMet, it shuttles back to SGs, where it accumulates Kedersha et al., 2000 ; . mRNAs within SGs are not degraded, making them available for rapid reinitiation in cells that recover from stress. The observation that labile mRNAs are stabilized during stress Laroia et al., 1999; Bolling et al., 2002 ; suggests that some aspect of the mRNA degradative process is disabled during the stress response. Thus, the accumulation of mRNA at SGs may be a consequence of both stress-induced translational arrest and stress-induced mRNA stabilization. Although the process of stress-induced mRNA stabilization is poorly understood, it likely involves the inactivation of one or more mRNA decay pathways. Two major mechanisms of mRNA degradation are active in eukaryotic cells Decker and Parker, 2002 ; . In the first pathway, deadenylated transcripts are degraded by a complex of 3 5 exonucleases known as the exosome. In vitro studies using cell extracts reveal that some mRNAs bearing adenine uridine-rich destabilizing elements AREs ; in their 3 untranslated regions are degraded by.
